- Evaluating person-centred care is the final phase in the process of planning and delivering care. This process of evaluation is explored throughout the chapter in the context of person-centred care. Evaluation processes involve the nurse or midwife working in partnership with the person and family to review the complete process of person-centred care. It is the ongoing process of judging the effectiveness of the planned care that has been delivered to meet the person's identified needs and goals. The purpose of evaluating care delivered is to assist the person to achieve his or her identified goals: promote health, event disease and illness, restore health, and facilitate coping with altered functioning. Evaluating care may be a formal process though quality improvement or Practice Development that are discussed in this chapter or informally through individual reflection by the nurse or midwife.
